We offer comprehensive assessments and geriatric care management to evaluate an older adult in their current living situation and provide recommendations and referrals.
What is a geriatric care manager?
A geriatric care manager is a professional advocate and coordinator for seniors and their families, helping to navigate the complexities of elder care. I specialize in providing comprehensive assessments, creating and managing care plans, coordinating medical appointments and services, and offering support for caregivers. My main goal is to help seniors maintain independence for as long as possible while providing their family members and caregivers peace of mind.
I started out my career in healthcare in 2002 after graduating from the University of Chicago with my Masters in Social Work. I have worked in hospitals, home care agencies and with guardianship clients. I also enjoy teaching undergraduate and graduate students at the Metropolitan State University of Denver.
I offer a free consultation over the phone! If you are looking for a one-time assessment and referrals I am happy to provide this. Some examples of issues I can address are home safety evaluations to determine the level of care needed, assisting families with long distance caregiving, collaboration with other medical professionals, and monitoring a client’s care in a senior community. I can also provide ongoing geriatric care management services. I have extensive experience and connections in the Denver Metro area with home care agencies, senior living communities and guardianship and fiduciary services for clients.
